Returning home after a hospital stay can feel overwhelming for both the client and the family. Even when discharge happens quickly, recovery often continues at home, where mobility is limited, routines are disrupted, and the risk of setbacks can be high. Inter-Coastal’s Post Hospitalization & Mobility Support Program is designed to support safe recovery, rebuild confidence, and reduce the risk of hospital readmissions. Through recovery-focused home care, mobility support, daily assistance, and RN-guided monitoring, we help clients regain stability while healing in the comfort of home.
